Conditions for obtaining service

Incident must been have computerized registered in system of civil status and passports department .

Notes

• The request accept only from owner of the required certificate or one adult family members.
• Provide the request to any of department offices and in case  the information  of certificate did not entered into computer the transaction will transfer to the office that register Incident requiring certificate in it.
• Show the personal card/ Family Book on it blogger Incident requiring certificate in it / Passport for requester if it non-jordanian.

Procedures

• Visit the audience service desk then show required documents for audit and approval then fill  the application form of obtain the certificate and get the turn number

• Visit  admissions officerto match the information submitted with the application and acceptance it in system by duly.

• Visit the office head  or his assistant to approval of the transaction

• Visit treasurer to pay legal fees.

• The transaction will send to the archive section to archive the transaction.

• The transaction will send to the printing section to print the document.

• Stamping and signed the document

• Delivery the certificate for the service recipient after the financial voucher handover to delivery employee.

•    Family Book for Jordanians or identity card that shows on it national number.
•    Form of disclose address
•    The identity card for Jordanian applicant 
•    Passport for Jordanian applicant .
•    Previous birth certificate, if there.

Jordanian citizens Foreigners whom got previously registered of Birth Incident.
