For the persons who are born in 1989, they should bring a suspended military conscription or a final exemption certificate.

Procedures
- The applicant shall visit Health Professions Licensing Department, fill out the designated application and submit the enclosed documents.
- The documents are verified and the application form is approved.
- A date is scheduled for the examination of the professional license (scheduled four times a year).
- One the exam is passed, the applicant pays the due fees in the Financial Department.
- The data of the applicant will be entered to the Professional License Issuing Program.
- The professional license is issued.
- The Minister of Health or his/her representative signs the license.
- The applicant receives the Professional License.
Required Documents
- An approved and certified copy of the General Certificate of Secondary Education certificate.
- An approved and certified copy of the (BA) bachelor degree.
- An approved and certified copy of the comprehensive diploma in Opticiancy and Optometric, and a certified copy of a training course in lenses from an accredited institute or college for a year, approved and certified by Ministry of Higher Education and Scientific Research of Jordan.
- A certified copy of the comprehensive diploma in Opticiancy.
- A non-criminal record certificate.
- A copy of the Civil Status ID card.
- Two (2) personal photographs.
Persons benefiting from the service
- Holders of BA in Optics.
- Holders of Comprehensive Diploma in Opticiancy and Optometric (duration of study is three years).
- Holders of Diploma in Opticiancy (duration of study is two years).
